We do not collect payment card numbers, Social Security numbers, or full financial account information through SMS.

- Written service agreements, admission packets, and consent forms.
- Email, telephone calls, and in-person conversations with our staff.

We use the information you provide for legitimate business and care-coordination purposes, including:

- Scheduling, confirming, rescheduling, or canceling appointments and visits.

4. SMS / Text Messaging Program
If you choose to provide your mobile number and opt in, we may send you SMS text messages related to your services with our agency. The types of messages may include appointment reminders, care-coordination follow-ups, and general two-way communication with your care team.
Message frequency varies based on your needs and the services you receive. Message and data rates may apply based on your mobile carrier’s plan.
Mobile Information — No Sharing
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All categories of sharing described in this Privacy Policy exclude text messaging opt-in data and consent; this information will not be shared with any third party.
5. How We Share Information
We share customer information only as needed to operate our agency and deliver services. Specifically, we may share information with:
- Members of your care team and authorized agency personnel who need the information to coordinate or deliver your services.
- Service providers and business associates who support essential operations (such as electronic health record vendors, scheduling platforms, billing processors, and our SMS messaging provider) and who are contractually obligated to protect your information.
- Government agencies, accrediting bodies, or other parties when required by law, regulation, subpoena, or to protect the safety of our patients or staff.
We do not sell customer information. Customer information is never shared with third parties or affiliates for marketing or promotional purposes. Information sharing for legitimate business operations does not include sharing of mobile opt-in data or text-message consent.
6. How to Opt Out of Communications
You may opt out of our SMS messaging program at any time:
- By replying STOP (or UNSUBSCRIBE, QUIT, CANCEL, or END) to any text message we send you. You will receive one final confirmation message and will then be removed from the SMS list.
- By calling or emailing our office at the contact information below to request removal.
- By unsubscribing from our email communications using the unsubscribe link contained in those emails.
Opting out of SMS will not affect our ability to provide services to you or to communicate with you through other channels (such as phone or mail) as needed for your care.

We use administrative, technical, and physical safeguards designed to protect the personal information we collect, consistent with applicable laws including HIPAA where it applies to protected health information. While no method of transmission is fully secure, we work to maintain reasonable protections and to limit access to those who need it.
